It is a legal document that states how you want future medical decisions to be made in the event you are unable to speak for yourself due to serious illness or injury or mental health issue.
Making an Advanced Directive. It is best to speak with an attorney about devising an Advanced Directive document in which you designate someone to be your representative, or agent, who will make the decisions about all aspects of your health care in the event that you are unable to make or communicate decisions about life and or death health care treatment. It also establishes what decisions they are able to make on your behalf.
In the most basic form, a health care power of attorney merely says, “I want this person to make decisions about my health care if I am unable to do so.”
